I see TONS of wedge pillows, many full length under mattress risers, but very few wedges that are about the same size as a large wedge pillow that go under the mattress. I would think these might be preferable to a wedge pillow – same elevation of the head and upper body, but without the sliding down and keeping more of the body on one’s actual mattress. They also would seem to avoid the feeling of sleeping downhill I experienced with a riser. Is there a reason more people don’t opt for this route.

Great question.
In my personal experience, the occasional downside to an under-mattress wedge is mattress slipping. Having to pull the mattress back up on the frame can be so annoying, and as such, I think people ultimately go from pillow wedge to adjustable frame for less overall hassle + improved convenience.
